RiskWatch Mobile 1.2.1

The RiskWatch mobile application supports conducting assessments on the go with a focus on clarity and usability. Note that access is restricted to active RiskWatch user accounts.

1. Figure 1 – Login Page: The user enters their username, password, and domain name. The domain name is specific to each customer and can be determined from the URL used to access the RiskWatch platform.

2. Figure 2 – Survey Progress: A pie chart displays the number of assigned surveys by status (Complete, Not Complete, In Progress). The most recently active survey is shown below the chart for quick access.

3. Figure 3 – Survey List: After selecting the pie chart, the Survey List page presents the assigned assessments. Each entry shows the facility name, end date, and progression status, and can be tapped to begin the corresponding survey.

4. Figure 4 – Take Survey: The page presents the assigned questions, which can be answered via text input, multiple choice, scales, or a review flag to mark questions for later review. A search bar at the top enables filtering of questions.

5. Figure 5 – Attachment: On the Take Survey page, tapping the upload button allows a photo to be uploaded from the photo library or captured with the device camera.

6. Figure 6 – Question Set Selection: The top-right hamburger menu provides the option to choose a different set of questions. Sets are color coded: green for completed, yellow for in-progress, and red for not started.
